In order to download applications you must have Adobe Reader installed on your computer.


Thank you for considering Jericho Commission. You may download one of two applications:  Aftercare Application--Ex-offender or Aftercare Application--Drug Court, complete it, and mail it to: Jericho Commission, PO Box 3067, Springfield, MO 65808.


Please note:


  • You must be referred by your PO, case worker, pastor, or other authority.
  • The application must be fully completed to be processed.
  • Allow forty-five days for completion of the process. You will receive a letter acknowledging receipt of your application.
