1.  you are seeking a mentor, complete this application and mail it to Jericho Commission.

Aftercare Application - This application is the initial requirement to connect  with a mentor in the Jericho Commission re-entry program.

2. If you are submitting an an aftercare application and you belong to a church, print this form and have your pastor complete it and mail it to Jericho Commission.

Pastoral Referral - This form is for pastors who are referring an applicant.

3. If you're incarcerated and submitting an aftercare application, ask your chaplain to print this form, complete it, and submit it to Jericho Commission with your aftercare application.

Religious Services Director's Appraisal - This form is for prison chaplains and religious directors who are referring an applicant.

4. If you're applying to be a mentor or to do volunteer work with Jericho Commission, complete this application.  Also, print the Pastoral Referral form, have your pastor complete and mail to Jericho Commission.

Mentor/Volunteer Application - This is the initial requirement to be a mentor or a volunteer in the Jericho Commission re-entry program.

5. If you want to partner with Jericho in supplying resources for our ministry, complete this application.
